Assessment, Treatment and Aftercare Planning

Each client accepted to will have an individualized aftercare plan specifying services and referrals needed upon discharge.

Continuing aftercare plans shall be collaboratively developed by counselors and clients prior to treatment discharge. Centerboard Wellness Center staff will work closely with the client and appropriate community agencies to ensure continuity of care is addressed and Colorado state statute requirements are met.

Each client at Centerboard Wellness Center will have an Initial Discharge Plan developed by that client’s designated counselor during assessment. The Initial Discharge Plan will focus on the individual needs of the client. Also, participation will include available family members, significant others and related community agencies.

It is recognized that discharge planning will be a continual process during a client’s treatment and that changes are likely to occur in the discharge plan.
